How to give a petition when change of a surname, name, patronymic name?
Change of surname, name, patronymic name by citizens of the Republic Kazakhstan is allowed on achievement them of 16-years old.
Change of surname, name, patronymic name and its registration are produced by the register offices according to an address of a proposer. Refusal about change of surname, name, patronymic name can be appealed in a court in installed by the law periods.
An order of consideration of a petition about change of surname, name and patronymic name is adjusted by the clause 194 of the Law " About marriage and family” of the Republic Kazakhstan from December, 17, 1998.
When presenting a petition about change of surname, name, patronymic name by a proposer an identity card or a passport of a citizen of the Republic Kazakhstan (military man - a document, certifying a personality, and the person of 16-years old which did not have a passport, - a birth certificate) is presented.
In a petition about change of surname, name, patronymic name must be specified the following information:
- a surname, name, patronymic name of a proposer;
- selected surname, name, patronymic name;
- reasons of change of surname, name, patronymic name
